HOW TO REGISTER

​Please select the applicable REGISTRATION FORM and input the relevant information (E.g., Name, Address, Weekly Clinic, Camp, etc.).​​

Please take a moment to review the terms and conditions of the NHD Waiver at the bottom of the REGISTRATION FORM. ​
- Once it has been reviewed, you must check the box agreeing to the terms and conditions of the NHD Waiver before pressing SUBMIT.
- The “NHD Waiver” must be completed and submitted before stepping on the ice. Failure to do so could result in refusal of participation.
